Guidelines for using a Fort Zumwalt School District Agreement to Hold Harmless and Indemnify Form

1.         A Fort Zumwalt School District Agreement to Hold Harmless and Indemnify Form may only be used for insurance purposes when there are 20 or less participants in an activity or event.

2.         Each member of the group needs to read and understand the Agreement to Hold Harmless and Indemnify form before signing.

3.         Once the form has been completed, the coach or sponsor of the activity/event must have   form notarized. Agreement to Hold Harmless and Indemnify forms that are not notarized will not be accepted by the district.

4.         The completed and notarized Agreement to Hold Harmless and Indemnify form needs to be turned into the school office prior to first use.

5.         If throughout the year children are added or subtracted from the activity/event a new Agreement to Hold Harmless and Indemnify form will need to be completed, notarized and turned into the school office.

6.         Adult groups cannot use an Agreement to Hold Harmless and Indemnify form.
